The general purpose of this form is to both help you improve your survey and coordinate surveys at the University of Mississippi. In the form, you will be asked to provide the following information:
Title of your survey
Purpose of your survey
Your contact information
Copy of your completed survey
Copy of your recruitment email
Description of incentives, if applicable
Information pertaining to panel member characteristics (campus location, type of student)
Proposed date for initial survey recruitment and follow-up emails
Proof of IRB approval, if applicable
Plan for disseminating aggregate survey results
Requests for sample panels for surveys from individuals or organizations outside of the University will rarely be approved. However, a paper copy of the UM directory can be purchased from the Office of the Registrar.
